They occur in all age group, most often between the ages of 20 and 50 years.

1. What are PILES ?

2.Classification: 

Piles are of two types. These are differentiated by their position with respect to the dentate line. It is a line which divides the upper two thirds and lower third of the anal canal.

 
  • Internal piles
  • External piles
 

Internal piles:

Internal hemorrhoids originate above the dentate line. They are covered by columnar epithelium, which lacks pain receptors or internal piles occur from 2 to 4cm above the opening of the anus.  The internal ones are much more common in human.

They were classified into four grades based on the degree of prolapsed :-

  • Grade I : No prolapsed, just prominent blood vessels.
  • Grade II : Prolapsed upon bearing down, but spontaneous reduction.
  • Grade III : Prolapsed upon bearing down requiring manual reduction.
  • Grade IV : Prolapsed with inability to be manually reduced.

EXTERNAL PILES :

Occur below the dentate. They are covered proximally by endoderm and distally by skin, both of which are sensitive to pain and temperature, External hemorrhoids are called perianal hematoma.

3.SYMPTOMS OF PILES :-

  • Pain while defecating.
  • Itchiness around the anus.
  • The area around the anus may be red and sore..
  • Bright red blood after a bowel movement.
  • After going to the toilet, a feeling that the bowels are still full.
  • Mucus discharge when emptying the bowels.
  • A hard lump may be felt around the anus.
  • It consists of coagulated blood, called a thrombosed external hemorrhoid.
